End-to-end medical billing
Medical billing that follows every claim to a resolution
End-to-end medical billing is the work of turning each patient encounter into a correct claim, getting it accepted by the payer, and following it until it is paid, adjusted or appealed. RCMHUB360 runs that cycle for your practice inside your existing practice management system, with a single team accountable for every step.
Does this sound familiar?
- Charges sit unbilled for days because no one owns charge review.
- Claims bounce at the clearinghouse and the rejections are not worked.
- Your biller left, and nobody knows what is outstanding.
- You get a deposit total each month but no explanation of what was paid, denied or still open.

How the work flows
- 01
Charges arrive
Encounters close in your EHR/PM; we reconcile scheduled visits against charges and flag anything missing.
- 02
Claim review
Demographics, eligibility, authorization and code-level edits are checked before the claim leaves.
- 03
Submission
Claims go out electronically; acknowledgments are monitored the same day.
- 04
Rejections worked
Front-end rejections are corrected and resubmitted rather than left in a queue.
- 05
Follow-up
Unpaid claims are worked by age and dollar value, payer by payer.
- 06
Reporting
You receive a monthly summary and a list of anything that needs your team's input.
What we measure
Measured against your own baseline. We don't quote benchmarks or other clients' results.
- Charge lag
- Days between date of service and charge entry.
- First-pass acceptance
- Share of claims accepted by the clearinghouse and payer without rejection.
- Clean claim rate
- Share of claims paid on first submission without correction.
- Days in AR
- Average number of days it takes to collect a dollar of charges.
What we need from your practice
- User access to your practice management system or EHR billing module
- Clearinghouse access (existing account, or we help you set one up)
- Fee schedule and payer contract summaries where available
- A named contact at the practice for documentation questions
- Patient information is exchanged only through secure access you control — never through forms or email on this website.
End-to-end medical billing: common questions
Do we have to change our EHR or practice management system?
No. We work inside the system you already use where user access can be provisioned for our team. If your system cannot support an outside billing team, we will tell you during the assessment.
Who owns the claims and data?
You do. Claims are filed under your practice's NPI and tax ID, and your data stays in your systems. Our access is provisioned by you and can be removed by you.
What happens to claims that were already outstanding before we start?
Existing AR can be included as a defined clean-up project, or left with your current team. We scope it separately so ownership is clear from day one.
How is pricing set?
Pricing is custom and depends on specialty, monthly claim volume, payer mix and which services you include. We explain the pricing models we offer during the consultation and put the scope in writing.
