Denial prevention and appeals

Work every denial once — then stop it from coming back

A denial is a payer's decision not to pay a claim, or part of it, with a reason code explaining why. Denial management has two halves: appealing or correcting the denials you have, and finding the upstream cause — eligibility, authorization, coding, documentation or enrollment — so the same denial does not repeat next month.

Does this sound familiar?

  • The same denial reason shows up month after month.
  • Appeals are written from scratch each time, or not written at all.
  • Nobody can say which payer or which provider drives most denials.
  • Authorization denials surface weeks after the visit.

Scope of work

Exactly what this service covers — and what it doesn't — so responsibilities are clear before you sign anything.

Included

  • Denial categorization by reason code (CARC/RARC), payer and provider
  • Corrected claims and reconsiderations for correctable denials
  • Appeal letters with supporting documentation, reviewed by a person before sending
  • Tracking of appeal deadlines and outcomes
  • Front-end prevention: eligibility, authorization and coding edit checks
  • Monthly denial trend report with recommended fixes

Not included

  • Clinical peer-to-peer reviews (your provider participates; we prepare the file)
  • External review or litigation
  • Guaranteed overturn of any denial

You already did the work. A stuck claim shouldn't stop you from getting paid.

How the work flows

  1. 01

    Capture

Denials are captured from ERAs and EOBs as they post, not discovered later.

  2. 02

    Categorize

Each denial is grouped by root cause and assigned a next action.

  3. 03

    Act

Correctable denials are fixed and resubmitted; others are appealed with documentation.

  4. 04

    Track

Appeal deadlines and outcomes are logged by payer.

  5. 05

    Prevent

Recurring causes go back to the team that can fix them — front desk, coding or credentialing.

What we measure

Measured against your own baseline. We don't quote benchmarks or other clients' results.

Initial denial rate
Claims denied on first adjudication as a share of claims submitted.
Denials by root cause
Distribution of denials across eligibility, authorization, coding, documentation and enrollment.
Appeal overturn rate
Share of appealed denials later paid.
Denial-to-action time
Days from denial posting to first corrective action.

What we need from your practice

  • ERA/EOB access
  • Clinical documentation access for appeals
  • Provider availability for peer-to-peer reviews when requested by payers
  • Patient information is exchanged only through secure access you control — never through forms or email on this website.

Denial prevention and appeals: common questions

What is a good denial rate?

It depends on specialty, payer mix and how denials are counted. Rather than quote a benchmark, we measure your baseline in the first month and report the trend against it.

Do you use templates for appeals?

We maintain payer- and reason-specific appeal structures, but every appeal is completed and checked by a person against the actual claim and documentation.
